Post-Participation Survey

Please help Catalyst Foundation to evaluate the outcomes, strengths and weaknesses of the Wellness Gallery project and the "Ageing Well" photo competition by completing our Post-Participation Survey below.
You can assist us by completing our survey if you have participated in ANY part of the project, including:

• submitting photos to the competition,
• voting in the competition, 
• attending a Wellness Gallery community photoshoot or Ageing in Media presentation,
• following Wellness Gallery on Instagram,
• OR connecting with Wellness Gallery or its project officers in any other setting.

We really appreciate you taking your time to complete our survey. It should only take a few minutes and really helps us to ensure we are able to continue to provide services and projects that are successful and beneficial to our community.
